Innovation Cafe: Strategies for Addressing the Social Determinants of Health
What: This one-day event will engage health system leaders and key partners in peer-to-peer learning and networking to spread innovation, with the aim of addressing key social determinants of health (housing, trauma, early learning/early childhood education, food insecurity, diaper insecurity). Best practices in health system innovation and collaboration with community-based organizations will be discussed through cafe-style project presentations and plenary sessions. When: June 5, 8:30 a.m.-3:15 p.m. Where: Oregon Convention Center (777 NE Martin Luther King Jr. Blvd., Portland, OR 97232) Audience: CCO and clinic staff, community-based organizations/partners representing key topic areas (see above), tribes, and other health system leaders. Cost: No charge to attendees. A draft agenda will be posted shortly on the Transformation Center's website.
